I understand that massage therapy, bodywork, facials and spa treatments provide stress reduction, relaxation, relief from muscular tension, and improve circulation.

If I experience pain or discomfort during the session, I will immediately inform my therapist, so that pressure/ strokes may be adjusted to my level fo comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after my session.

I understand that todays' services are not a substitute for medical care. My therapist is not qualified to diagnose, prescribe or treat physical/ mental illness.

I affirm that I have notified my therapist of all know medical conditions, injuries and medications/ allergies.

I agree to inform the therapist of any changes in my medical condition, there shall be no responsibility, reliability on the therapist part shall I neglect/ decline to do so.

I understand that massage and any spa services/ treatments are therapeutic and non-sexual in nature.

I waive and release my therapist, Legacy Wellness LLC, dba Legacy Spa and Wellness, its partners and or affiliates from any liability; past, present and future, relating to my massage therapy, body work and/ or spa services and treatments.
